Temporomandibular joint Dysfunction (TMD) covers a range of disorders of the jaw joint and the muscles which support it. It can arise from many causes acting together, but mostly it is due to overuse of the joint and muscles.

The temporomandibular joint joins the lower jaw (the mandible) to the skull, just in front of your ear.

Symptoms of TMD

Many people suffer from headaches or neck problems for years without realising that it is actually coming from the muscles supporting the jaw. Pain can come from the jaw joint itself, the muscles that support the joint and neck, and even be referred to other areas such as your ears and/or eyes.

TMD Symptoms

Head

Headaches, sinus pain, shooting pains, scalp/hair sensitivity, temple pain

 

Eyes

Pain behind the eyes

 

Ears

Earache (without any ear infections), ringing in the ears, ‘itchy’ or full feeling in the ears

 

Teeth

Pain in the teeth and/or gums, sensitive teeth, teeth that feel loose, teeth that are wearing down and/or breaking

 

Jaw

Pain in the cheeks and jaw, clicking or popping sounds, jaw that locks, instability of the jaw

 

Neck

Aches and pains in the neck, back and shoulders, sore and/or tired muscles 

Causes of TMD

  • Tooth grinding/clenching (bruxism)
  • Poor posture and holding the jaws in certain positions
  • Habits such as biting fingernails or holding a pen between your teeth
  • Overuse of the muscles e.g., excessive yawning or biting into large foods like burgers
  • Trauma/injuries to the joint
  • Arthritis/degeneration of the joint

How do you treat TMD?

 

Unfortunately there is no good evidence surrounding why we grind/clench our teeth and no one -size-fits all approach to treating TMD. We can’t stop you from grinding your teeth. What we can do is manage the symptoms and try and prevent acute flare-ups. We have a number of treatment options at our fingertips:

1. Education and self-care: this is the most important step. Understanding your condition and learning how to manage it is key. At your TMD assessment appointment, we will go through strategies on how you can take care of your symptoms at home.

2. Exercises: we will prescribe at-home exercises for you to do

3. Referral to physio: we work with physios who have a special interest in head and neck. They can do manual therapy to help relax tense muscles.

4. Occlusal splints: we can make thin but highly strong and comfortable mouth-guards to be worn over-night to try and counteract the side effects of tooth grinding in your sleep (nocturnal bruxism). These help prevent the teeth from chipping and breaking due to tooth grinding and clenching.

5. Medications: in certain cases we may want to prescribe medications to relax muscles, or use neurotoxin injections to relax the muscles.

6. Referral to other specialists:

– Sleep physicians: there are links between nocturnal bruxism and obstructive sleep apnoea

– Maxillofacial surgeons: if we suspect problems with the jaw joint itself
